• New Patients: Please arrive 15 minutes before your appointment time to complete registration.
• Financial Responsibility: Patients are responsible for providing current and active insurance information at the time of each visit and are responsible for knowing what services their plan does or does not cover. Harvey Young, M.D. accepts most major group plan insurances and will bill all insurances on patients' behalf.  If the information given by the patient is inaccurate and denied, the patient will be responsible for the balance of the visit. Patients are responsible for the payment of co-pays at the time of service.  Patients are also responsible for paying any applicable co-insurance, deductibles and all other procedures or treatment not covered by their insurance plan. In the event the patient’s health plan determines a service to be “not payable”, the patient will be responsible for the complete charge and agree to pay the costs of all services provided.
• Cancellation Policy: Please give our office 2 business days notice to change or cancel an office visit or 5 business days notice to change or cancel a procedure to avoid a late cancellation fee:
  • $50 for office visits
  • $200 for procedures
